Forms required for prospective students:
UChicago Financial Aid Worksheet completed via the applicant’s UChicago Account (please note, UChicago does not accept the CSS Profile or documents submitted through IDOC)
Parent 2023 Federal Tax Return (including all schedules) and W-2 forms
Parent 2023 Federal Business tax returns (if applicable) including all schedules. These are required regardless of percentage ownership that parents hold in the business.
2025-2026 Free Application for Federal Student Aid (FAFSA) (School code 001774)
The 2025-2026 FAFSA is now available for students and families to complete. Please note, students applying in early admissions application plans (Early Action, Early Decision I, and Early Decision II rounds) will receive a financial aid estimate if all other requirements have been submitted by the financial aid priority deadline. However, a completed 2025-2026 FAFSA is required in order to finalize the financial aid estimate. PLEASE NOTE: We are currently unable to download completed FAFSAs. If you have completed a FAFSA and added UChicago, your checklist will not show it as received until we are able to download completed FAFSAs.
Prospective students must upload documents through their UChicago Account. This is the fastest, most efficient and most secure way to upload a variety of financial aid documents. Please review your UChicago Account for a list of documents that can be uploaded. The Office of Financial Aid cannot accept emailed documents.
Important Note for Transfer Applicants: Transfer Applicants must indicate interest in need-based financial aid prior to being admitted.
